3. Report an accident

Accidents will need to be reported back to the club as soon as practicably possible. Below is the accident report form and must be filled out and handed to Gabby Rowe so we have a record of the incident.

If it is a concussion or a suspected concussion, then after the accident form has been filled in the head coach must then follow the head case protocol.
